• Distance & tolls: loaded miles + deadhead assumption, plus tolls if route requires.
• Vehicle & staffing: ambulatory vs wheelchair van; door-through-door vs curb adds crew time.
• Wait policy: grace period included, then 15-min increments; staged pickup can reduce hospital standby.
• Timing: after-hours, holidays, and metro traffic buffers — book ~48h ahead Mon–Sat when possible.

How to save without shortcuts

• Check Medicaid / Medicare Advantage NEMT first — private-pay is backup when broker can’t meet window.
• Combine appointments on one day when clinically okay — one round trip vs multiple shorts.
• Be precise on vehicle level — wheelchair van < stretcher referral when stretcher not required per orders.
• Share shortest reasonable route; ask what’s included (base, mileage, tolls, wait, after-hours, cancellation).
• For dialysis/therapy recurring, ask about standing weekly schedule — same driver, predictable cost.
